The main services provided in the RSSC are the Progressive Care Programme, domiciliary ventilation, specialist oxygen systems and the management of a wide range of both respiratory and non-respiratory sleep disorders. The Progressive Care Programme (invasive ventilation weaning programme) was initiated in 1992. Patients who are failing to wean from invasive ventilation despite optimal conventional therapy on Critical Care units are referred to this service. A multidisciplinary team approach is employed to optimise therapy, and non-invasive ventilation is often used to allow a graded withdrawal of invasive ventilatory support. The service has been cited as a model for the future provision of Critical Care step down programmes.
Domiciliary ventilation is provided for patients with chronic respiratory failure due to conditions such as Chronic Obstructive Pulmonary Disease (COPD - chronic bronchitis, emphysema and bronchiectasis), scoliosis, Motor Neurone Disease, Obesity Hypoventilation Syndrome, muscular dystrophies, previous poliomyelitis and spinal cord injuries. Patients are assessed for their suitability for treatment, and long-term follow up is arranged for those started on ventilatory support. The RSSC currently cares for over 1,500 patients on ventilatory support in their own homes. The majority receive non-invasive ventilation but those requiring tracheostomy ventilation are also supported.
The RSSC provides a comprehensive assessment for all types of oxygen treatment, particularly portable systems for use outside the home and when oxygen is required in conjunction with home ventilatory support.
